HAZARD IDENTIFICATION AND RISK ASSESSMENTS

INCIDENT INVESTIGATIONS

Investigation of incidents is a vital part of every health and safety management system. No other activity pro- duces such quick results as the prompt reporting and investigation is to determine the causes, and put correc- tive measures in place to prevent a recurrence. Serious incidents or accidents with a high potential of injury or damage will require an in-depth investigation but every incident is a signal of problems that need to be correct- ed. An incident investigation program requires planning, preparation and training. It requires a commitment by the Employer that investigations will take place when required and appropriate to do so, and that recommen- dations from investigations will be acknowledged by management and implemented where possible and fea- sible.  Explain the purpose and benefits of incident investi- gation  Describe how to prepare for and conduct an investi- gation  Recognize immediate causes and develop effective recommendations for corrective actions to prevent recurrence of incidents At the end of this course you will be able to:

Occupational injuries and illnesses happen when work- ers are exposed to hazards in their workplaces. If you eliminate the hazards, or at least prevent workers from being exposed to hazards, then it follows that they will not have work-related injuries and illness. If the hazard cannot be eliminated, then the employer must do every- thing reasonably practical to reduce the level of risk faced by workers to the lowest amount possible. In order to determine if the hazards can be eliminated or the threat to workers reduced, the employer must assess and prioritize the level of risk posed by those hazards, and establish controls based on the level of priority.

At the completion of this course, participants will be able to:

 Understand the legislated requirements for conducting risk assessments;  Explain what a hazard is;  Explain what risk is;  Identify the process for identifying hazards and assessing risks;  Demonstrate ability to prioritize risks;  Recognize the hierarchy of control to effectively deal with hazards;
